Level 3 H&P (99223)

The 99223 represents the highest level of initial care for patients being admitted to the hospital. This is the most popular code used to bill for admission H&Ps among internists who selected the 99223 for 60.4% of these encounters in 2003 for a total of 2,392,266 visits. The 99223 ranked 42nd among the most frequently usedCPT codes by all physicians in 2003. The reimbursement for this level of care is approximately $154.00. Usually the presenting problems are of high severity.


The documentation for this code requires THREE out of THREE of the following:

1)    Comprehensive History
2)    Comprehensive Exam
3)    High Complexity Medical Decision-Making

Or 70 minutes spent face-to-face with the patient if coding based on time.  The appropriate documentation must be included.
