Effective January 1, 2023, this information is no longer up-to-date. The material on this page covers only the 1995 and 1997 E/M guidelines and is no longer accurate. A new set of E/M guidelines was released in 2021, with some minor modifcations added for 2023. These new guidelines are now used to document all encounters in both the outpatient and inpatient settings. For the most recent E/M coding guidance, visit our home page here.

It's okay to use documentation templates, as long as you know what you're doing.  We are proud to offer self-auditing E/M documentation templates for the following visits:

  • New Office Patients
  • Established Office Patients
  • Initial Hospital Care (Admission H&Ps)
  • Subsequent Hospital Care (Hospital Progress Notes)

These templates are available in mutliple versions using the different specialty exams from the 1997 E/M guidelines.

The General Multi-System Exam version of the New Office Patient Self-Auditing Template® is available free of charge to users who register for our free content.

All other templates are offered free to physicians who sign up for our full curriculum on E/M coding courses.
