The 99222 represents the second highest level of initial care for patients being admitted to the hospital. This is the second most popular code used to bill for admission H&Ps among internists who selected the 99222 for 35.5% of these encounters in 2003 for a total of 1,407,842 visits. The 99222 ranked 61st among the most frequently used CPT codes by all physicians in 2003. The reimbursement for this level of care is approximately $110.75. Usually the presenting problems are of moderate severity.
The documentation for this code requires THREE out of THREE of the following :
1) Comprehensive History
2) Comprehensive Exam
3) Moderate Complexity Medical Decision-Making
Or 50 minutes spent face-to-face
with the patient if coding based on time. The appropriate
documentation must be included.
