What are Medicare carriers?

Answer: Medicare carriers are private insurance companies who contract with the federal government to administer the Medicare program in various states. Carriers are responsible for paying Medicare claims in their region and, among other things, for auditing physician notes to make sure the documentation supports the level of care reported.
