Can you mix and match elements of the 1995 and 1997 E/M guidelines in the same noter?

Answer: Apparently that's a big no-no. Several Medicare carrier websites go out of their way to state that, in theory, you should not mix and match elements from the 1995 and 1995 E/M guidelines within the same note.
