What are the E/M guidelines?

Answer: The E/M guidelines are the official rules recognized by Medicare (and most private insurers) for documenting physician-patient encounters. Two sets of guidelines were released, one in 1995 and the other in 1997. Physicians may use EITHER the 1995 or the 1997 guidelines when documenting their encounters. The choice of which version to use is left entirely up to the provider. Auditors are told to review charts with both sets of rules in mind and to use the version which is more beneficial to the provider when deciding upon the documented level of care.
