If an encounter has more than one qualifying risk element, which level of risk do I choose?

Answer: The overall level of risk is determined by the highest element of risk present. This is explicitly stated in the E/M guidelines as follows:

"The highest level of risk in any one category (presenting problem(s), diagnostic procedure(s), or management options)
determines the overall risk."
