A hospital progress note...

 

CC: F/U CHF exacerbation

Denies any spontaneous somatic complaints.

BP 130/80, HR 72, RR 22.  Conversant.
Lungs are clear with normal respiratory effort.
Heart is RRR.  Improving lower extremity edema.

Labs are normal except for sodium of 129.

Impression

  1. Improving CHF exacerbation.
  2. Well controlled HTN.
  3. New finding of mild hyponatremia.

Plan

  1. Stop DIURIL.
  2. Continue FUROSEMIDE.
  3. BMP in a.m.
