CC: F/U CHF

Interval History: No new complaints offered.

Exam: NAD. 158/76, 72, 20. Lungs have bibasilar crackles. Normal respiratory effort. Heart is RRR. There is worsening symmetric bipedal edema.

Chem 7 and CBC WNL.

Impression

1. Worsening CHF exacerbation
2. Poorly controlled hypertension due to hypervolemia

Plan

1. Increase LASIX to 40 mg PO BID.
2. Repeat chem. 7 in a.m.
